Low-frequency, high severity instances make for the best press and the worst resource allocation. PETA is using that for emotional manipulation--which is especially laughable given that none of these incidents have been of especially high severity.
On the day in Utah in 1999 where there was a non-fatal carriage incident (the most severe of the three), statistically there would have been a 35% chance that somebody died in a motor vehicle accident, and the probably that at least one person would have died in a MVA over a five-day period is 98.9%. (Poisson distribution, using L = 330/365 based on this document.)
Lest PETA try to use the possibility of an MVA-horse carriage incident, the horses are working on city streets in which cars are generally travelling at low speeds, and again, the incidents are very rare. By far and away the greatest threat to pedestrians, cyclists, and drivers is...other vehicle drivers. By PETA's logic, the use of motor vehicles around horses is cruel and inhumane, not to mention the threat to other users of the road such as cyclists and pedestrians. Shouldn't we ban the motor vehicles?

Perhaps they don't allow bleeding hearts into their city.
We had a case here not long ago that PETA went into an uproar about. While it was rather hot, an older horse dropped dead in the harness while hauling a wedding party in the downtown district. The mayor was/is apparently conducting a survey as to whether or not they should shut them down. I say shut down the mayor and PETA! They're all idiots..
