Need Reading Chart Help: INTV and SUR?

Forgive my ignorance I'm used to reading harness horse charts; I have an old copy of a horse's past performance record I found in Jockey Club microfiche at our local Agricultural College, and there's a couple of columns I don't understand, most are pretty self explanatory except these two columns for me.

It lists the RACE IDENTIFICATION, WT, PST, FIN, and something labeled INTV, then continues CLAIM PRICE, EARNED, DIST RACE, then something called SUR, PURSE, RACE DESCRIPTION, NO RNR, TOP CLAIM, TIME, TRK CND, TRK REC.

Can somebody tell me what the columns labeled INTV and the numbers listed below it means?(was that the horses odds). Also another perplexing column labeled SUR that usually is blank but sometimes has a T, anybody know the meaning of this?

Well, unlike harness horses, as you well know, thoroughbreds sometimes run on TURF... and if designated under "Sur", one would assume that "Sur" means "Surface".

you should help us solve the other one by showing the numbers designated there as written.

OK, the SUR (surface) category now makes perfect sense, thanks!

Under the INTV column it always lists a four digit number:
