Sabino spot that thought it was on the other side of the horse. Little girl could have received a kick or a scratch or anything and it could be scarring.

I found a picture of it from when he was younger. It was there, but much smaller... so it seems it's growing. It was already white in that younger photo and he wasn't very old there so I kinda doubt scarring. It's not on the other side of the withers either.

Looks like a bite mark to me, upper and lower jaws.

I agree, except that in the older photos it was just two small dots (one above and one below). Not only that, but I've seen a TON of bite marks and have yet to see one scar this badly.

We have a chestnut filly who developed a round white spot just below her withers when she was 4-5 months old. Six months later, she got another spot, this one above a hock.
Neither was in response to an injury, or really to anything at all. They just appeared one day. Maybe his did, too.

My buckskin filly, who is now 2 going on 3, has a white spot on her back. It is kind of almost an O. It has been there since she was little, no injury.

Here's a picture of a tb that we owned a few years ago and he was always getting these "white spots." His show name is Chance of Fluries... registered name is Simple Billie. I was told that the spots were caused by a vitamin deficiency as a foal/yearling: looks like this foal could be experiencing the same issue.

Those are Birdcatcher spots. I don't know if they are ALWAYS caused by that deficiency or if some of them just occur, but most of the time they're just called Birdcatchers.
I've known two horses in the past year who never had a spot develop spots like that ALL OVER. One of them was perfectly healthy and never had an issue and the vet said "they're Birdcatchers, they happen". The other one got sick and the vet said "they're caused by..." something that starts with a "t". They both just look like normal Bircatchers to me. *shrug*
